Pernicious access

Definition

Serious but rare, sometimes fatal event, occurring after the plasmodium falciparum infestation (most often), parasite transmitted by the female Anopheles mosquito responsible for malaria.

Symptoms

Access is characterized by a syndrome (set of symptoms) pernicious (very serious) which is frequently brutal:

  • Falanga (the patient moves more)
  • Prostration (extreme weakening of muscle forces forcing the patient to stop)
  • Severe asthenia (fatigue)
  • Collapse (inability of some organs function normally: this is the case among other blood circulation and the cœur)
  • Digestive problems (nausea, vomiting, diarrhea) originally of dehydration (fluid loss) resulting…
  • Anuria (absence of urine)
  • Hyperazotémie (elevation of nitrogen from the destruction of the proteins of the patient in the blood)
  • Cold sweat
  • Cyanosis (staining tissues and the skin blue-purple reflecting the decrease in the arrival of oxygen)
  • Intense cooling of the extremities (hands and feet)
  • Hyperthermia (elevated temperature) important
  • Hypoglycemia (decrease of the rate of glucose in the blood)
  • Severe anaemia

Evolution of the disease

Disorders of consciousness and seizures can occur, up to and including the coma. Initially, it’s simple tremors, delirium, and a very high fever. In this case, the prognosis of the disease is less favourable.

Treatment

It uses quinine injected directly into the blood by intravenous hospital. Without properly driven treatment, mortality is 20%. The patient may also submit final neurologic sequelae, especially the child.
