Acrodermatitis Continua Suppurativa Hallopeau

Definition

Dermatopathy (skin disease) consisting of erythematous vesicles (small droplets pink or red kind), containing a liquid oily or Pustular, in fingers or on all of the hand, extending sometimes to the other ends (plants of the feet) and is accompanying a breach of the nails (inguinal dystrophy). Most often, the Acrodermatitis of Hallopeau is located at the level of the thumb.

Symptoms

Step or little pain history of local trauma. Pustular lesions (pus crusts) sometimes located under the nail. Association to a psoriasis (eruption of Red plaques covered with scales).

Evolution of the disease

By relapses for years: with territories remain red and there is atrophy (decrease volume) of the epidermis (surface layer of the skin). Sometimes, lesions start at a finger then gradually earn the neighbouring fingers. Relapses, which renew themselves continually, eventually interfere much with the patient. In some patients only, atrophy eventually win vicinity tissues, i.e., nails and bones, leading to a risk of destruction of the nail, or even the underlying OS.

Treatment

The use of cortisone based cream more often gives insufficient results. Some patients respond to the local used or the use of tablets (vitamin A acid) Retinoic acid. The used is a therapeutic method that is called also photo-chimiothérapie. It combines exposure to ultra-violet rays and the administration of psoralen (substances a photosensitising action, i.e. to an increase in the sensitivity of the skin to radiation, especially ultraviolet, used in this technique). This increased sensitivity of keratinocytes (epidermal cells) is possible through the blocking of their multiplication by psoralens which bind DNA component chromosomes. The absorption of psoralens is accompanied by a small amount of milk approximately 2 1/2 hours prior to the meeting orally. It lasts from 1 to 20 minutes, during which the subject (undress) is a pair of goggles, and genital protection when he is a man. The number of session from 2 to 3 per week, and then gradually decreases to 1 per week for a few months. Contraindications to the used are:

  • Renal failure (kidney filters operating deficit)
  • Pregnancy
  • Cataract
  • Liver failure (loss of liver function)
  • The skin cancer
  • Precancerous lesions
